Highest case fatality of any infectious disease
- Prodromal phase - fever, nausea, vomiting, headache, fatigue, tingling at site of wound
- Furious phase- agitation, disorientation, seizures, hydrophobia - can’t treat past here
- Dumb phase - paralysed disoriented, stuporous
- Progress to coma then death
